Mumbai, October 10, 2022: Early intervention at Fortis Hospital, saved the life of a 63-year-old patient, who experienced a rupture in the swelling of the largest artery. Mr. Pulchand Jaiswar, an Indian Railway employee, was diagnosed with ruptured abdominal aortic aneurysm, a life-threatening condition that causes bulging or swelling of the aorta to burst. The severity of the condition resulted in limited supply of oxygen to bowel and kidneys. Mr Jaiswar’s condition became more critical due to the dysfunctional left-side kidney. Due to these co-morbidities. There was a high mortality risk associated with his condition.

Aorta is the largest blood vessel, which supplies blood to the entire body, from the heart. In this case, the elderly patient witnessed pain due to an enlargement in the organ. This enlargement was further exacerbated after an outburst of the swelling that resulted in bleeding in the stomach.

Keeping in mind the sensitivity of Jaiswar’s condition, the doctors performed an emergency endovascular aneurysm repair (EVAR) surgery on the patient at Fortis Hospital, Mulund. EVAR potentially holds greater advantage for high-risk patients with multiple comorbidities as it is minimally invasive. Among the team of doctors were Dr Manish Hinduja, Consultant-Cardio Thoracic & Vascular Surgery, Dr. Girish Warawdekar, Radiologist, Dr. Rahul Pandit, Director-Critical Care, and Dr. Vijay Shetty – Anesthesiologist.

Explaining about the surgery, Dr Manish Hinduja said, “If a patient with a ruptured aneurysm is not treated in time, then there is more than a 50 per cent chance of the patient dying in less than a week. In this patient’s case, he came to us on time and got the necessary medical attention.”

“As he had multiple health problems we had to opt for EVAR. As, if the cases would have gone for routine surgery, then both the chest and abdomen would have had to be opened, and it would have carried a high risk of pre-operative mortality. Hence, we opted for the minimally invasive surgery,” he added further.

Elaborating on the challenges associated with the surgery, Dr. Girish Warawdekar said, “The challenge in EVAR was that this aneurysm had multiple branches, so a single covered stent wouldn’t suffice. Hence, prior to the surgery, we invested an hour to design a special graft that had a two-wire-enforced opening for placing separate grafts in the right kidney and intestine, so as to restore the flow in these two organs.”

The minimally invasive surgery carried out by Fortis Hospital allowed the patient to recover quickly and walk on the second day, ready for discharge the next day. He had a one-inch incision, which made his recovery faster and the pain tolerable. Additionally, his kidney function improved quickly, post-surgery.

The patient Pulchand Jaiswar said, “I was not sure my condition was critical until I visited Fortis Hospital. The doctors immediately diagnosed my health problem and started the treatment swiftly. I was worried that it would take long for recovery post-surgery, but I was able to go home in merely three days and was able to resume work soon.”
