1. Discounts are not applicable on outsourced tests and advanced testing
2. Services are subject to availability only
3. Services are subject to prior appointment booking
4. Discounts are only available on cash payment basis and not applicable on TPA/insurance-routed cases
5. OP discounts are applicable on Consults and Investigations only
6. IP discounts are applicable on room rent and diagnostics & excludes drugs, consumables, implants, and packages
7. Discounts on Preventive Health packages are on Fortis standard packages only
8. Offers/schemes cannot be clubbed
9. Services can be availed by dependant members on production of card copy along with relationship proof through a valid government id card only
10. One-time waiver of Registration charges for cardholders only at the location of Fortis Hospital selected
11. Card benefits will be valid for a period of 3 year from date of issue of the card.

13. The decision of Fortis management will be final and binding in case of any disputes.
