Understanding Butt Acne: Causes, Treatments and Prevention Strategies

The skin has small openings called pores that can get clogged with oil, bacteria, dead skin cells and dirt, leading to pimples. If you frequently experience pimples, especially multiple at once, you might have acne, a skin condition that causes these breakouts.

Butt acne is often mistaken for regular acne but is typically not the same. It may result from conditions like folliculitis, keratosis, pilaris or boils.

What causes butt acne?  

Many different conditions can lead to pimples on the butt, resulting in a lengthy list of potential causes. However, there is an ideal ecosystem that can contribute to these issues. Let’s explore the butt acne causes:  

  • Folliculitis: When it comes to pimples on the bum, the most frequent cause is folliculitis. In simple terms, folliculitis refers to the irritation of a hair follicle. That irritation might stem from any number of problems. It can be something as simple as friction, wearing clothes that are too tight, shaving too often or too close, waxing or even plucking hair. Folliculitis can also be caused by a bacterial infection (like staphylococcus aureus), a fungal infection (a type of yeast infection of the skin) or even a mite. Individuals with weakened immune systems, particularly those living with HIV and AIDS, may develop less common types of folliculitis, such as eosinophilic folliculitis. Physicians typically manage eosinophilic folliculitis using anti-retroviral medications. Folliculitis can vary in appearance depending on its cause and severity. Sometimes, it looks like a single pustule, while other times, it appears more like a rash.
  • Keratosis pilaris: It occurs when the protein keratin accumulates in hair follicles. This condition is common among younger individuals and typically fades with age. It becomes more noticeable when the skin is very dry, especially during winter months.
  • Boils (carbuncles): Boils are a particular form of folliculitis. Deep folliculitis is commonly referred to as a boil, and the medical term for a single boil is a furuncle. When there are multiple boils or if they have several white heads, we refer to that condition as a carbuncle. Boils are typically caused by an overgrowth of the bacteria known as Staphylococcus aureus. Approximately 20% of people carry staphylococcus aureus in their noses or on their skin. These bacteria can transfer to under your fingernails and spread to other areas of the body. This is why regular handwashing and refraining from popping pimples is important. Although uncommon, fungi, streptococcus and pseudomonas can also lead to painful abscesses.
  • Contact dermatitis: It refers to skin inflammation caused by direct contact with an irritant or allergen. Eczema is one of the most prevalent forms of dermatitis.
  • Eczema: It is a common condition that leads to dry, itchy and scaly patches on the skin, and it can occur almost anywhere on the body. In some cases, eczema may also produce a bumpy rash that looks similar to pimples.
  • Sweating: This issue particularly affects athletes, especially student-athletes, who often have to remain in their sweaty uniforms for hours before getting home to shower.
  • Friction: Consider the clothes you choose to wear, especially focusing on the fabric and how they fit. It might be time to set aside the athleisure look for something that offers better breathability.
  • Sitting: If your body remains relatively inactive, the sweat and bacteria produced throughout the day can become trapped on your bum.
  • Shaving, waxing and depilating can irritate your hair follicles, leading to inflammation.

Treatment

There are various reasons for blemishes on the buttocks. Often, it is due to bacteria, but it could also be related to yeast or even a viral infection. Identifying the underlying cause is crucial, and your healthcare provider can help you determine what is going on.

If you have butt acne that does not go away with basic care measures, it may be time to consult a dermatologist. A dermatologist will be able to assess your condition professionally and create a butt acne treatment plan based on your needs, such as prescription medications, topical treatments or procedural treatments in more severe cases.

Prevention

Preventing butt acne is easier said than done, but there are several steps you can take to minimise the likelihood of it coming back.

  • Taking a shower: Shower right after exercising. If that’s not feasible, at least change your clothes.
  • Wash regularly and with care: Showering daily can be beneficial for many reasons beyond just preventing butt acne. It is one of the key lifestyle changes you can adopt to avoid breakouts. However, try to avoid scrubbing your backside with harsh exfoliates. A gentle wash with soap and water or one of the recommended cleansers is less likely to irritate your skin.
  • Choose non-comedogenic moisturisers: While occlusive moisturisers are useful, they should not be applied to your butt. Instead, opt for oil-free, non-comedogenic products that will not block your pores.
  • Move around regularly: Sitting for long periods, whether by choice or necessity, increases the chances of skin problems. Taking breaks to stand up and stretch or incorporating brief exercises throughout the day can make a difference.
  • Maintain a healthy diet: We have all heard that what you consume affects your skin. This applies to your peach’s skin as well. Make sure to stay hydrated by drinking plenty of water and try to reduce your intake of dairy, high-carbohydrate and sugary foods daily.
  • Choose cotton: While everyone enjoys a comfy pair of underwear, cotton underpants are best for preventing pimples. If you dislike basic styles, select breathable materials and steer clear of thongs.
  • Avoid tight-fitting clothing: Friction can worsen butt acne, so opt for loose, breathable garments instead of yoga pants and skinny jeans.
  • Avoid popping pimples: Although it might feel satisfying, popping pimples is not a good idea. Most likely, it is not just a simple pimple (and you should not pop those either). If you have folliculitis or a carbuncle, you risk spreading or worsening the infection. Even worse, you could transmit it to someone else.

Keeping the area clean, choosing the right underwear, exfoliating regularly and avoiding picking at acne can all help reduce its occurrence.
