Ludhiana, May, 2025: In a rare and complex case, doctors at Fortis Hospital, Ludhiana has given a new lease of life to a 20-year-old student, who had presented with a massive 24-cms-cyst in her right ovary and was in severe abdominal pain for three consecutive days. A Laparoscopic Ovarian procedure was performed under the guidance of Dr. Arti Tuli, Senior Consultant, Obstetrics and Gynecology, Fortis Ludhiana with support from the hospital’s radiology team. Despite the complexity of the case, doctors were able to preserve her ovary. She recovered well post her surgery and was discharged in stable condition the very next day.

The Patient, a resident of Ludhiana, had initially consulted a local physician and was prescribed painkillers. However, her condition got worse prompting an emergency visit to Fortis Hospital Ludhiana. She arrived late at night, experiencing severe abdominal pain and extreme discomfort, which had persisted for past three days. Though her vitals were stable, she was in significant distress. Initial tests including a CT scan and tumor markers were conducted. The scan revealed a giant 24-cmscyst in the abdominal cavity. Due to its massive size, the exact ovarian origin could not be initially determined, but it was later confirmed during surgery to be in the right ovary. The cyst had caused ovarian torsion, a painful and potentially fertility-threatening condition.

The team opted for a laparoscopic ovarian cystectomy, a minimally invasive approach rarely used for cysts of this size. The surgery lasted approximately one hour and involved carefully removing the cystic fluid during the procedure to reduce its volume and safely remove it through small keyhole incisions.

Dr. Arti Tuli, Senior Consultant – Obstetrics and Gynecology, Fortis Hospital Ludhiana, explained, "Given the patient’s young age, unmarried status, and the benign nature of the cyst as confirmed by negative tumor markers, we opted for a laparoscopic surgery. This approach is rarely used for cysts of this magnitude, as most surgeons opt for open surgery. However, with careful planning, radiological consultation, and aspiration of cystic fluid to reduce its size during surgery, we were able to proceed laparoscopically. This ensured minimal scarring, preservation of the ovary and a faster recovery. This case highlights how modern laparoscopic techniques can be a boon for young women. Even large ovarian cysts can be safely removed with minimally invasive surgery, provided it is appropriately planned. My advice to young girls is to not ignore signs like abdominal bloating, discomfort, or changes in menstrual patterns. Early consultation and simple ultrasounds can prevent long-term complications and protect long-term reproductive health.”
