-Endoscopic Ultrasound (EUS) Guided Coiling and Glue Injection was used to treat the critically ill patient-

Kolkata, June 2025: A team of doctors from the Department of Gastroenterology at Fortis Hospital, Anandapur, Kolkata- Dr Arnab Sarkar, Dr Tuhin Mitra, and Dr Debasish Datta successfully treated a critically ill-patient suffering from alcoholic hepatitis which was further complicated by gastrointestinal bleeding, via an advanced minimally invasive technique known as Endoscopic Ultrasound (EUS)-guided coiling and glue injection.

A 42-years old male patient with a long-standing history of alcohol abuse, first visited the OPD at Fortis Anandpur with complaints of progressive jaundice, weakness, abdominal swelling, and pedal edema. On initial evaluation, he was diagnosed with alcoholic hepatitis, and endoscopy revealed the presence of GOV2 gastric varices (abnormally dilated veins in the stomach that carry a high risk of internal bleeding.)

Since the patient showed no active signs of bleeding at that time, the team opted for medical management of jaundice using steroids. However, the patient missed follow-up for about a month and returned with black stools (melena)– a sign of internal gastrointestinal bleeding. His condition had worsened significantly, with hemoglobin dropping to 6.5 g/dL, indicating severe blood loss.

Sharing insights on the case, Dr Arnab Sarkar, Consultant, Gastroenterology, Fortis Anandapur, Kolkata said, “On medical re-evaluation, we found that the gastric varices had increased in size. Given the recent bleeding and high risk of further hemorrhage, we decided to proceed with EUS-guided coiling and cyanoacrylate glue injection, a more advanced and targeted intervention. A procedure used to treat bleeding gastric varices, where coils are placed under EUS guidance, followed by cyanoacrylate glue injection to obliterate the varices and prevent rebleeding.”

During the procedure, the doctors used an endoscopic ultrasound to precisely locate the affected veins. Tiny metallic coils were placed to block the blood flow, followed by injection of medical-grade glue to permanently seal the varices. This technique is known for its high success rate and lower risk of re-bleeding, especially in high-risk patients.

Following the procedure, the patient showed rapid clinical improvement. Follow-up endoscopy confirmed a significant reduction in the size of the varices, and there were no further signs of bleeding.
