57-Year-Old Woman Undergoes Successful Robot-aided Surgery for Vaginal Prolapse at Fortis Noida

Noida, 24th April 2025: Doctors at Fortis Hospital Noida have successfully treated a 57-year-old woman suffering with severe vaginal prolapse via a robot-aided surgery. The patient had been suffering from severe vaginal prolapse for over 20 years, following a uterus removal surgery. Vaginal prolapse is a condition, when the top of the vagina (also called the vaginal vault), sags and descends into the vaginal canal. This condition arises due to weakening and stretching of the pelvic floor muscles, tissues, and ligaments, often leading to organs shifting from their normal position

The complex procedure was performed using robot-assisted surgery by a multidisciplinary team led by Dr. Shailendra Goel, Director, Uro-Gynaecology and Robotic Surgery, Fortis Noida. The surgery lasted two hours, and the patient was discharged in stable condition within two days.

Upon admission at Fortis Noida, the woman was in significant pain, experiencing blood-mixed pus-like discharge from her prolapsed vagina. She had difficulty passing urine and walking due to the prolapsed mass between her legs. An MRI was conducted which revealed a stage 4 vaginal vault prolapse with presence of hernia of the urinary bladder, small intestine and rectum through the vagina. Looking at the condition, the team of doctors then planned a robot-aided Sacro-colpopexy, a minimally invasive vaginal lift surgery where a special medical net (surgical mesh) is used to attach the vaginal vault to the sacrum (lower backbone). Performed through four small keyhole incisions in the abdomen, the procedure was completed successfully despite the multiple co-morbidities faced by the patient. The patient also suffered from co-morbidities - The chronic nature of the ulcers (wounds) on the prolapsed vaginal tissue and adhesions involving the small intestine, urinary bladder, rectum and additionally from COPD and asthma.

Giving details of the case, Dr Shailendra Kumar Goel, Director - Uro-gynecology and Robotic surgery, Fortis Hospital Noida said, "Vaginal prolapse is a common condition among middle-aged and elderly women, particularly post the removal of the uterus. This is mostly caused by weakening of the pelvic floor muscles. It can result in urinary and bowel dysfunction and if left untreated, may lead to other complications such as ulceration, infection and fistula formation. This was a challenging and complex case due to the long-standing nature of the prolapse and involvement of multiple pelvic organs. Robotic assistance played a crucial role in ensuring precision during dissection and mesh placement, particularly in areas affected by adhesions of urinary bladder, small intestine and rectum. The Da Vinci Xi Robot-aided system provides high-definition 3D vision and superior instrument control, making it especially effective for complex urogynecology procedures like this.”
