74-Year-Old-Man Successfully Operated for life-threating Cyst Extending from Sinus Extending Brain & Eye at Fortis Escorts Faridabad

-Fluid Filled Cysts in the sinus with extension into brain and eye are a rare occurrence and reported in only 2.4 in 1 million people-

Faridabad, 21st January 2025: Doctors at Fortis Escorts Hospital Faridabad have successfully removed a rare fluid-filled cyst from a 74-year-old man, which was located on his sinus, extending to his brain and left eye, thus severely compromising his vision. The team of doctors led by Dr Surender Kumar, Consultant, ENT, Fortis Escorts Hospital Faridabad conducted a challenging surgery to remove the mucocele (fluid-filled cyst), relieve pressure on the eye socket, and repair the leak of cerebrospinal fluid from the brain. The procedure was done in approximately 2 hours, and the patient was discharged in a stable condition within 2 days. Mucoceles are rare in the general population, occurring in 2.4 in every 1 million people. 

The patient had developed the cyst a year back and had previously undergone surgery for the cyst at another tertiary care hospital in the city 7–8 months back, but there was no improvement. He then came to Fortis and upon admission, CT–PNS (Paranasal and sinusis) and MRI of his brain was conducted, which revealed mucocele (fluid-filled cyst) with a large extension into the eyes, brain and the surrounding bones, which is quite rare. Post evaluation, an endoscopic excision of the mucocele was conducted on the patient, along with decompression on the left eye and repair of cerebrospinal fluid leak from the brain. The surgery was successful, and the patient had a gradual recovery.

Giving details of the case, Dr Surender Kumar, Consultant, ENT, Fortis Escorts Hospital Faridabad said, “Such a rare condition occurs due to chronic sinus infection when the ostium of sinus (small openings that connect the sinuses to the nasal cavity) gets obstructed, which leads to formation of mucocele. In this case, the patient had a high-risk of permanent loss of vision, as the swelling was compressing on the optic nerve. Secondly, there was fluid-filled swelling, due to which there was significant loss of bones of the head due to erosion, and there was high risk of cerebrospinal fluid leak from the brain and chances of getting meningitis and other complications. However, with the right treatment approach, the patient was successfully treated. If he was not treated on time, the cyst would have extended into his brain, compressing over the vital structures of the frontal part of the brain with neurological and cognitive deficit and high chances of permanent loss of vision from left eye.”
