Rare Case: 6.5 kgs Diseased Kidney Surgically Removed from 57-Year-Old Dialysis Patient at Fortis Anandapur, Kolkata 

~The patient was suffering from Autosomal Dominant Polycystic Kidney Disease (ADPKD)~ 

Kolkata, May 2025: In a complex and high-risk surgery, Doctors at Fortis Hospital, Anandapur successfully removed a 6.5 kg kidney from a 57-year-old man suffering from Autosomal Dominant Polycystic Kidney Disease (ADPKD). ADPKD is a genetic condition where the kidneys enlarge massively and stop functioning — Patient, Mr. Joydeep Chandra was undergoing dialysis for end-stage-kidney disease at a city-based hospital, where he was referred to Fortis Hospital Anandapur on April 28, 2025 following repeated episodes of internal bleeding and severe abdominal pain.  

Patient Mr. Chandra had been living with ADPKD for several years, however recently , the cysts in his left kidney began to rupture that led to  internal bleeding. In one such instance, nearly 1.5 litres of blood had to be drained from his abdomen. With no signs of improvement in his health condition and escalating complications related to the condition, the Nephrology and Urology teams jointly decided to perform a left nephrectomy, surgical removal of the kidney. 

The surgery was performed by a multi-disciplinary team of doctors comprising of Dr. Shrinivas Narayan, Director, Urology, Fortis Hospital, Anandapur, on April 29, 2025 and Dr. Arup Ratan Dutta, Director, Nephrology, Fortis Hospital, Anandapur, who had been managing the patient’s dialysis and kidney-related complications since 3 years. 

Normally, a human kidney weighs about 150 - 200 grams and measures 11–13 cms in length. In this case, the left kidney had expanded to nearly 50 cms and weighed 6.5 kgs, occupying almost the entire abdominal cavity on one side. Since the diseased kidney occupied the entire abdominal cavity on one side, minimally invasive procedures like laparoscopy or robotics were not feasible, thus an open surgery was performed. 

Dr. Shrinivas Narayan, Director, Urology, Fortis Hospital, Anandapur, said, “I have performed several kidney removal procedures, but this was a rare and complex case. For patients on dialysis, such surgeries are high-risk due to the potential for heavy bleeding and damage to nearby organs. However, in this case, the kidney had become a source of recurrent hemorrhage and persistent pain. Its surgical removal provided the patient with immediate relief and significantly improved his quality of life.”  

Despite the risks, the procedure was successful. The patient recovered well post-surgery and was discharged within 5 days. The patient continues to be on dialysis and has been advised to consider a kidney transplant to improve long-term outcomes, as his right kidney is also polycystic and could present similar complications in the future. 

Dr. Arup Ratan Dutta, Director, Nephrology, Fortis Hospital, Anandapur, added, “The patient had non-functioning kidneys with repeated infections and abscess formation. As part of the nephrology team, we made the clinical decision to proceed with nephrectomy to prevent further complications. Surgical intervention was critical in this case, and timely coordination with the urology team helped ensure a safe outcome. As his right kidney is also significantly enlarged and prone to future episodes of hemorrhage or infection, he may require a second surgery before he can proceed for kidney transplantation.”
