The Department of Anaesthesiology is one of the most critical and multifaceted specialties in modern medicine. While it may not always occupy the spotlight, the anaesthesiologist is present at the most vulnerable moment of a patient's surgical journey — ensuring they remain safe, comfortable, and pain-free from the very first breath of anaesthesia to the last moment of recovery.
Far beyond simply "putting a patient to sleep," anaesthesiology today is a sophisticated, evidence-based discipline that demands an in-depth understanding of physiology, pharmacology, critical care, and pain medicine. The Department of Anaesthesia serves as the backbone of every surgical specialty — from cardiac and neurosurgery to orthopaedics, oncology, obstetrics, and paediatric procedures — making it one of the most wide-ranging and indispensable departments in any tertiary care hospital.
The Role of the Anaesthesiologist
An anaesthesiologist is a fully qualified medical doctor with specialized postgraduate training in anaesthesia, critical care, and pain management. Their responsibilities span the entire perioperative period — the time before, during, and after surgery — and extend well beyond the operating theatre.
- Pre-operative Assessment — Before any elective procedure, the anaesthesiologist conducts a thorough evaluation of the patient's medical history, current medications, allergies, organ function, and overall fitness for anaesthesia. This assessment is crucial in identifying and mitigating risks, particularly in patients with complex comorbidities such as heart disease, diabetes, respiratory conditions, or renal impairment. A personalized anaesthesia plan is developed for every patient based on this evaluation.
- Intraoperative Management — During surgery, the anaesthesiologist is the sole clinician whose undivided attention remains on the patient at all times. They administer and titrate anaesthetic agents, monitor vital parameters including heart rate, blood pressure, oxygen saturation, end-tidal CO₂, and depth of anaesthesia — making real-time adjustments to ensure the patient remains stable, unconscious (or appropriately sedated), and free from pain throughout the procedure.
- Post-operative Care — After surgery, the anaesthesiologist oversees the patient's safe emergence from anaesthesia in the recovery room, managing any immediate post-operative complications such as nausea, vomiting, shivering, or pain. Effective post-operative pain management is central to faster recovery, improved patient satisfaction, and reduced hospital stay.
Types of Anaesthesia
The choice of anaesthetic technique is tailored to the nature of the surgery, the patient's medical profile, and the preference of the surgical team.
- General Anaesthesia — The patient is rendered completely unconscious and unaware during surgery through a combination of intravenous and inhaled anaesthetic agents. General anaesthesia is used for major surgical procedures involving the abdomen, thorax, brain, or spine, as well as for patients who cannot cooperate or tolerate other forms of anaesthesia.
- Regional Anaesthesia — Rather than rendering the patient unconscious, regional anaesthesia numbs a specific region of the body by blocking the nerve supply to that area. This includes spinal anaesthesia (injection into the fluid surrounding the spinal cord), epidural anaesthesia (injection into the epidural space, commonly used in labour and major lower limb or abdominal surgeries), and peripheral nerve blocks (targeted injection near specific nerves using ultrasound guidance).
- Regional anaesthesia has gained significant traction in modern anaesthetic practice due to its superior pain control, reduced opioid requirements, earlier mobilisation, and faster recovery compared to general anaesthesia in many surgical contexts.
- Local Anaesthesia — Used for minor procedures, local anaesthesia numbs only a small, defined area of tissue without affecting consciousness. It is commonly used for skin procedures, dental work, and minor day-care surgeries.
- Monitored Anaesthesia Care (MAC) / Sedation — For procedures that are minimally invasive or diagnostic in nature — such as endoscopies, bronchoscopies, or interventional radiology procedures — the anaesthesiologist provides carefully titrated sedation, keeping the patient relaxed and comfortable while maintaining spontaneous breathing and protective reflexes.

- Cardiac Anaesthesia — Managing anaesthesia for open-heart surgeries, valve replacements, coronary artery bypass grafting, and heart transplants demands an exceptional understanding of cardiovascular physiology and haemodynamic management. - Pain Management — Chronic pain — whether arising from cancer, post-surgical syndromes, nerve injuries, or musculoskeletal conditions — is a growing public health challenge. Anaesthesiologists specializing in pain medicine offer a comprehensive range of interventional and pharmacological strategies, including nerve blocks, epidural steroid injections, radiofrequency ablation, and spinal cord stimulation.
