A Complex Case of Recurrent Pelvic Tumor: A Triumph of Surgical Excellence

Managing recurrent pelvic tumors presents significant challenges, requiring precise diagnosis and multidisciplinary intervention. Here, we share the case of a 62-year-old male who underwent a complex surgical procedure after battling persistent abdominal pain and recurrent tumor growth.

The Patient’s Journey

Two years ago, the patient underwent a laparoscopic low anterior resection with loop ileostomy at another center. His post-operative recovery was uneventful, and he resumed his normal activities without complications. However, six months ago, he began experiencing persistent lower abdominal pain that did not subside with routine treatment.

Diagnosis and Initial Management

Upon evaluation, his tumor markers were initially within normal limits, and a CT scan of the abdomen revealed a pelvic abscess. He was treated with antibiotics, which offered temporary relief. However, despite five months of medical therapy, his symptoms persisted. A repeat assessment revealed a concerning rise in tumor markers—exceeding 100—indicating a possible recurrence. A PET-CT scan confirmed the presence of a recurrent tumor with necrosis, infiltrating the prostate and seminal vesicles.

A Complex Surgical Approach

Given the extent of tumor infiltration, a total pelvic exenteration was performed. This involved the removal of the affected pelvic organs, including the bladder and rectum, necessitating an end-sigmoid colostomy and ileal conduit for waste elimination. The procedure was meticulously planned and executed to ensure complete tumor clearance while maintaining the patient’s postoperative quality of life.

Post-Surgical Recovery and Future Course

The patient is currently recovering well following surgery. To optimize his treatment outcome and prevent further recurrence, he will undergo second-line chemotherapy and radiotherapy targeting the tumor bed. This combined approach aims to enhance long-term disease control and improve his prognosis.
