Case of situs inversus (reversed organs) is extremely rare and occurs in approximately 1 in 10,000 individuals
Faridabad, 10th June 2025: In a rare and high-risk case, doctors at Fortis Escorts Hospital, Faridabad successfully performed a complex laparoscopic gallbladder surgery on a 70-year-old man suffering from acute calculus cholecystitis with biliary peritonitis - a serious condition in which a gallbladder infection caused by gallstones leads to rupture and bile leakage into the abdominal cavity.
What made this surgery particularly challenging was the patient’s diagnosis of situs inversus, a rare congenital condition occurring in approximately 1 in 10,000 individuals, where the internal organs are positioned in a mirror image of the normal layout. In this patient, his liver and gallbladder were on the left side instead of the right, while the spleen and heart were on the right, creating a completely reversed anatomical scenario for the surgical team. A team of doctors led by Dr. Mohsin Khan, Senior Consultant, Department of General Surgery at Fortis Escorts Hospital Faridabad, successfully performed a minimally invasive laparoscopic cholecystectomy, removing the infected gallbladder while preserving other surrounding vital organs. The patient was discharged after two days of the surgery in stable condition.
The patient was admitted at Fortis Escorts Faridabad with severe abdominal pain, along with nausea and vomiting. He had a prior history of Type 2 diabetes and coronary artery disease and had undergone angioplasty. His heart’s pumping ability was significantly weakened, with only 30% efficiency compared to the normal range of 50 -70%. An ultrasound was conducted at the hospital, which revealed acute calculus cholecystitis, along with signs of biliary peritonitis - bile poisoning, which could quickly lead to multi-organ dysfunction, if left untreated.
Given the patient’s narrowed heart arteries, he was on blood thinners to prevent clots, increasing the risk of excessive bleeding during surgery. After a carefully executed two-hour procedure, the severely infected gallbladder was successfully removed, preventing a life-threatening situation.
Giving details of the case, Dr. Mohsin Khan, Senior Consultant, Department of General Surgery at Fortis Escorts Hospital Faridabad said, “This was a critical and rare case because an infected and inflamed gallbladder had ruptured and spread infection in the body, leading to a life-threatening condition like sepsis. Additionally, the patient had situs inversus, a rare congenital condition occurring in about 1 in 10,000 individuals, which made the surgery highly challenging. Any error could have resulted in severe complications. Through timely and precise surgical intervention, we were able to safely remove the gallbladder and prevent further risks.”
Yogendra Nath Awadhiya, Facility Director, Fortis Escorts Hospital Faridabad said, “This was a highly complex and rare case that required exceptional surgical expertise and meticulous planning. Under the leadership of Dr. Mohsin Khan, our team of doctors navigated these challenges with precision, ensuring the best possible outcome for the patient. At Fortis Escorts Hospital Faridabad, we are committed to handling intricate and high-risk cases with advanced medical expertise and state-of-the-art technology.”

The incident took place at the child's home and the family immediately rushed him to the emergency Department at Fortis Escorts Faridabad. In a rare and crucial move, they also brought the alive snake with them in a jar, which helped the doctors immediately identify it as a cobra snake and begin the right treatment without delay.

The emergency team acted swiftly, administering life-saving anti-snake venom and placed the child on ventilator to support his breathing.
